Oelwein Blueprint for Health

The Iowa Chronic Care Consortium is engaged in an active model program to build a blueprint for improving community health in Oelwein, Iowa. ICCC's commitment to population health improvement requires experience in the construction of community based plans that can be replicated across a variety of local communities. Planning work began during the past three months with surveys and assessments of interests, needs, problems and resources. A 15-member leadership group has been developed. Financial resources from the local community and other funders will be utilized. Oelwein is a model Iowa community that has been dramatically affected by the meth problem. In fact, a non-fiction book of national prominence has been written about the community and its meth problems. ICCC likes to begin where others fear to tread.

Healthy Living Ambassador Program

ICCC is partnering with the leadership of Healthy Polk 2020 in launching a pilot program to develop visible, articulate advocates of good health in central Iowa. The goal of developing advocates is to create leaders who will inspire others to take personal responsibility for individual health. They will serve as resources to model responsible healthy living and to engage others to be responsible for their individual health management. As this pilot program is implemented and evaluated, the intention is that it can be replicated in other communities with other sponsors. At the core of the program curriculum will be the key pillars of healthy living. As a part of this program, ambassadors will be expected to complete a personal plan, a community plan, and a service project. The program is expected to launch in the spring of 2011.

Health Risk Assessments

ICCC has worked at the community level with Ankeny Health Futures (a Harkin Wellness Grant recipient), Oakridge Neighborhood Services, and community groups in Des Moines to deploy Health Risk Assessments to a range of populations - from youth to seniors. ICCC provides consultation to community groups and others on the deployment of health risk assessments and the planning of health improvement activities for their populations.
