William Appelgate, PhD

As Executive Director of ICCC, Dr. Bill Appelgate has provided leadership and guidance in the areas of chronic disease management, clinical health coaching, health risk assessments, health policy, prevention, health promotion and healthy aging.
Under his leadership ICCC has led the Iowa Medicaid Enterprise in deploying statewide chronic disease programs in heart failure and diabetes to its members. He was also actively involved in the development of chronic health care cost reduction strategies within recently enacted health care reform legislation.
Dr. Appelgate speaks nationally on the topics of chronic disease management, population health management and health policy. He has addressed organizations such as the American Telemedicine Association, the Care Continuum Alliance (formerly the Disease Management Association of America or DMAA), The Center for Telehealth and E-Health Law, and the AT&T North Texas Health System Think Tank.
His early professional work included service as a director of a university research center, a college foundation director, a community college president, and CEO of two comprehensive and innovative healthcare systems in Wisconsin and Iowa. During the 1980s he received recognition as the Outstanding New Professional in Aging Services in North America from the American Association of Homes and Services for the Aging. Most recently, Dr. Appelgate served a ten-year term with Des Moines University as its Vice President for Planning & External Relations. His areas of responsibility included planning, marketing & communications, information technology, state and federal government relations, community relations, institutional research, accreditation and quality advancement, development and alumni relations.
Priscilla Gilbert
Wellness Program Coordinator | Email
Priscilla Gilbert is a trained and certified health and wellness coach and the Wellness Program Coordinator for ICCC. In this role Priscilla works with small businesses in developing employee wellness programs, presenting wellness topics and providing individual health coaching. Priscilla serves as a faculty member for the Clinical Health Coach™ Training Program, where she assists in instruction on health coaching techniques.
She also serves as a consultant to ICCC for community programs and initiatives that are involved in developing community health assessments and using individual health risk assessments (HRAs) to determine the health status of a community or group.
Previously, Priscilla was Membership Manager with the Wellness Council of Iowa where she also served as project manager for the Modeling Wellness for Youth Program. The program involved conducting health risk assessments and developing physical activity and nutrition programs for youth in five school districts. Priscilla's education is in health promotion and elementary and special education.
Jody Hereford, RN, BSN, MS, FAACVPR

Jody Hereford has served as Clinical Project Consultant for ICCC since summer of 2010 and has been a primary consultant in the development of the curriculum and design of content for the Clinical Health Coach™ training program. She also serves as faculty for the program in health coaching techniques in the clinical setting, clinical care management and leadership.
Jody, who resides in Boulder, Colorado, is a certified Health Coach, a Registered Nurse/Exercise Physiologist and has completed a Cardiovascular Fellowship through the American Hospital Association's Health Forum. She has worked with hospitals as an expert in quality patient care including chronic illness care and reducing patient complications, readmissions and avoidable deaths. Jody has published extensively and spoken frequently on topics including innovative programmatic redesign, current business models and policies and procedures.
With a broad base of experience in the field of cardiopulmonary health, rehabilitation and management services, she has been involved with hospital and community-based programs, residential disease prevention programs and corporate- based employee health and rehabilitation services. She also is a Past President and Fellow of the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), was the inaugural Chair of their Disease Management Task Force, served on their Committee for Telemedicine/Telehealth, and is also past chair of the Education Committee.
Currently Jody continues as the founder and principal in a consulting practice that provides education, expert resources and design work in the field of health management services. Her passion centers around creating health for all individuals where they live and work through effective patient engagement, health promotion and chronic care management strategies.
Kathleen Kunath, RN

As Clinical Project Manager for ICCC, Kathy Kunath leads disease management demonstrations, and is involved in community health improvement projects and prevention programs utilizing health risk assessments (HRAs). She currently serves as project coordinator for the Clinical Health Coachâ„¢ training program and "Keep It in Check," the Iowa Medicaid Diabetes Tel-Assurance Program. She also serves on the Care Continuum Alliance (formerly DMAA) Medicaid Guidelines WorkGroup.
Kathy was instrumental in the development of the Physician Office Health Coach Training Program, which was the predecessor to the Clinical Health Coach™ training program and conducted in partnership with Mercy Clinics, Inc., from 2008-2010.
As an independent consultant in project management with a career focus of promoting and supporting healthier individuals and communities, Kathy has worked for ICCC since 2004. She is certified as a Program Director through the American College of Sports Medicine, and as an administrator of the Myers-Briggs Personality Type Indicator™.
Kathy has 24 years experience in the field of cardiovascular healthcare, cardiac rehabilitation and health promotion. In 1999, she was chosen by the American Hospital Association's Health Forum as one of 30 participants across the U.S. to participate in a Cardiovascular Health Leadership Fellowship. In 2003, she was commissioned by the Iowa Department of Public Health to lead a team in writing the white paper, "Chronic Diseases: A Critical Issue for Iowa." Kathy has also served in leadership roles for numerous state coalitions, including Tobacco Free Iowa, Iowa Cardiovascular Risk Reduction Coalition and Iowa Association for Cardiac and Pulmonary Rehab.
Jennifer Moulton, RPh
Educational Programs Coordinator | Email
Jennifer Moulton has served as Educational Programs Coordinator for ICCC since April 2010. In this role she has assisted in the development and design of the Clinical Health Coach™ training program and coordinated the program's requirements for continuing education and professional development. She also serves as faculty for health coaching to improve medication adherence.
As a registered pharmacist, Jennifer is Chief Operating Office for the Collaborative Education Institute (CEI), an organization that offers quality, outcomes-focused education to pharmacists in Iowa and nationally. She has spearheaded a five-state pilot initiative to introduce a continuing professional development (CPD) program and process; and completed a contract through the Pharmacy Quality Alliance to develop and disseminate educational activities regarding pharmacy performance measures. Through CEI, Jennifer develops and oversees live, self-study and web-based continuing pharmacy education activities in the areas of preceptor development, pharmacy quality, immunizations, pharmacy technicians, and pharmacy law.
Jennifer is owner of Moulton Education, LLC, which specializes in continuing pharmacy education and patient education projects. She has particular interests in examining medication adherence strategies with individuals through health coaching.
Shirley A. Roberts, MPH

As program consultant for ICCC since 2008, Shirley Roberts serves in project management and program development for clinical and community-based projects. Currently she is the project leader for the Pre-Medicare Health and Wellness Program for Rural Iowans, a joint project with the Iowa Farm Bureau Federation. She also serves on the Population Health Management Task Force for the Care Continuum Alliance (formerly DMAA).
As an independent consultant in healthcare project management, Shirley is also currently serving as project manager for the Des Moines University (DMU) Mobile Health Clinic. The mobile health clinic allows DMU to provide inter-disciplinary medical services throughout the community.
Shirley has over 20 years of experience in the healthcare delivery and insurance businesses. As the first service line administrator for the University of Michigan Medical Center's Heart Care Program, Shirley was instrumental in assisting the medical center in improving access to and quality of heart care services. During this time, she participated actively with the healthcare and business community through the Greater Detroit Area Healthcare Consortium's heart care initiatives to address quality and practice variation issues.
Also while at the University of Michigan Medical Center Shirley expanded her role in clinical practice assessment as Associate Administrator in the Office of Clinical Affairs, working with clinicians in establishing tools to analyze costs and quality of services. Subsequent to this role, she joined Wellmark Blue Cross and Blue Shield as a process improvement consultant. She assisted in the strategic planning of process management at Wellmark and the implementation of Six Sigma and other process improvement tools.
Shirley has a Masters of Public Health (MPH) and a Masters of Music degree from the University of Michigan.
